Welcome to Transition Coordination
AdvantAge Health Resource Center is your Montrose, Delta, and Gunnison Counties Transition Coordination (TC) Options Counselor.
TC is a program of the Area Agency on Aging and your local Aging Disabilities Resources for Colorado (ADRC) that provides two types of services: Options Counseling and Community Transition Services/Extended Transition Services. These services are directed and governed by the Colorado Department of Health Care Policy and Financing (HCPF).
The TC program is designed to help facilitate clients who want to transition from nursing or long-term care facilities back to the community using home and community-based services and supports. Options counseling is the first transition coordination (TC) process step.
When a nursing home resident is interested in exploring options to return to the community, we provide your residents with the following:
- Conducting an initial interview with the resident to gauge interest in transitioning
- Educating the resident on the available community programs to aid in transitioning: Transition Coordination (TC), Home & Community-Based Services (HCBS), and the Program for All-Inclusive Care for the Elderly (PACE)
- Preparing the referral to send to the resident’s selected Transition Coordination Agency (TCA)
Under their Long Term Care Medicaid benefit, any resident living in a skilled nursing facility can request an Options Counseling meeting. A family member can also make referrals. A referral to options counseling does not mean the resident is committing to leave a facility. It is simply a request for information on community-based service options.
